Online forms

We use Netcall Liberty for our online forms. See our Digital Customer Service programme for more details. The web team can produce these forms for you, and can work with ICT to integrate the form submissions with other systems in most cases. See request an online form.

We work with Research and Consultation to use Smart Survey for forms which are used to gather opinion, or where you need to analyse and produce reports of the results.
